Laser Lit Review April 2017

Compiled by Dr Igor Cernavin, Prosthodontist, Honorary Senior Fellow University of Melbourne School of Medicine, Dentistry and Health Sciences, Director and Cofounder of the Asia Pacific Institute of Dental Education and Research (AIDER), Australian representative of World Federation of Laser Dentistry (WFLD).

Gojkov-Vukelic et al1 examined the effects of treatment of hypersensitive dental cervix with diode laser and found that laser therapy proved to be an extremely safe and effective treatment of cervical dentine hypersensitivity.

Caccianiga and coworkers2 carried out a study to assess if low-level laser therapy enhances the efficiency of orthodontic dental alignment. They concluded that the administration of LLLT might significantly increase the efficiency of orthodontic treatment during dental alignment.

Rosa et al3 carried out a study to verify the accuracy of laser fluorescence for caries detection in vitro and concluded that it had the ability to diagnose occlusal carious lesions in permanent teeth and enamel and dentin caries.

Giannelli and coworkers4 investigated the in vitro efficacy of photodynamic treatment (PDT) with methylene blue (MB) photoactivated with lambda 635nm diode laser and of lambda 405nm violet-blue LED phototreatment for the reduction of bacterial biofilm and lipopolysaccharide (LPS) adherent to titanium surface mimicking the bone-implant interface. They found that non-invasive phototherapy was an attractive alternative in the treatment of peri-implantitis to reduce bacteria and LPS adherent to the titanium implant surface without causing damage to surface microstructure.

Santinoni et al5 conducted a systematic review to evaluate the effectiveness of low-level laser therapy (LLLT) in enhancing maxillofacial area bone repair. They concluded that improvement in bone density can be found when LLLT is applied postoperatively in maxillofacial bony defects. LLLT also seems to promote anti-inflammatory and analgesic effects and accelerate healing, as well as enhance quality of life related to oral health. However, LLLT use protocols need to be standardized before more specific conclusions can be drawn about this subject.

Menem and coworkers6 tested the diagnostic accuracy of a pen-type laser fluorescence (LFpen) device in detecting approximal carious lesions, in posterior permanent teeth, at the cavitation and non-cavitation thresholds, and compared it with that of digital bitewing radiography. They found that the LFpen's diagnostic performance was accurate and significantly better than digital bitewing radiography in detecting approximal caries lesions, in posterior permanent teeth.

Bayer et al7 compared laser and ozone treatment on oral mucositis in rats and concluded that LLLT is more effective than ozone.

Martins and coworkers8 assessed the efficacy of photobiomodulation (PBM) in the recovery of inferior alveolar nerve (IAN) injury in rats following crushing injury and found that IAN-injured rats which received PBM had a significant improvement of IAN morphometry when compared to IAN-injured rats without PBM.

Comunian et al9 examined the effects of LED and low-intensity laser on the healing of rabbit dental alveoli. Their findings were that photobiomodulation with LASER and LED produced effects, but only the LED demonstrated a beneficial effect.

Javed and coworkers10 carried out a literature review to assess the effectiveness of diode lasers in the management of orofacial pain and concluded that diode laser therapy is more effective in the management of oro-facial pain compared with traditional methods.

Gisler11 wrote an interesting paper explaining how even “hopeless” periodontally involved teeth with very deep pockets of 10mm or more can be saved using laser without open surgery or antibiotics.

Li, Zongbo12 examined the cleaning ability of Er:YAG laser against Enterococcus faecalis in root canals and concluded that it can be effectively used for root canal disinfection without NaClO and Ca(OH)2.

Valente et al13 carried out a review of the literature to assess the increase of temperature following laser irradiation with 810 nm, 980 nm, and 1064 nm diode laser wavelengths, of an implant under conditions that more closely replicate those of the human body. Their conclusion was that use of these diode lasers does not cause a harmful increase in temperature when used under conditions similar to those of the human body.
