Laser Lit Review February 2019

Latest Literature Review

The latest literature review has been prepared by Dr Igor Cernavin and you can download it at the link below or read the text below in this email.

Compiled by Dr Igor Cernavin, Prosthodontist, Honorary Senior Fellow University of Melbourne School of Medicine, Dentistry and Health Sciences, Director and Cofounder of the Asia Pacific Institute of Dental Education and Research (AIDER), Australian representative of World Federation of Laser Dentistry (WFLD)

Alikhasi et al1 published an interesting and useful article about the sensitivity and specificity of laser fluorescence (DIAGNOdent) for detecting residual excess ZnO-based cement around dental implants. Their conclusion was that the residual cement was recognized by gently walking the device tip around the implant. A detection score below 16 indicated an absence of cement and scores of 16 or higher indicated the presence of excess luting agent in the implant sulcus. The sensitivity and specificity of DIAGNOdent to detect REC in the sulcus were 100% and 96.67% respectively. Based on their findings, they propose that DIAGNOdent could be used to detect REC in the sulcus of cement-retained implant supported restorations.

Nejem Wakim and coworkers2 described a study of decontamination of Dental Implant Surfaces using the Er:YAG Laser Beam and found that the decontamination of the implant surfaces with a low-energy Er:YAG laser with three passages, appeared to be an encouraging approach.

Miloro et al3 used a prospective, double-blind, randomized controlled clinical trial, to determine whether the proportion of nerve-injured patients with postoperative neurosensory improvement over a 3-month period differed significantly between a control group and a low-level laser therapy (LLLT) group. Their study failed to provide sufficient evidence to conclude that a difference in neurosensory improvement exists between the LLLT and placebo groups with inferior alveolar nerve or lingual nerve injuries.

Zabeu and coworkers4 evaluated the bonding strength of non-simplified dentin bonding systems DBS to dentin irradiated with a diode laser (970 nm) immediately and after 12 months of water storage following either primer or bond application. They concluded that laser treatment of non-simplified DBS was not able to stabilize their bonding characteristics after 12 months.

Rozza-de-Menezes5 evaluated the efficacy of four methods to prevent chemotherapy-induced oral mucositis in patients with solid tumors. Forty-eight patients, aged 27-84, were randomly allocated to different groups from the first day of chemotherapy (CT), in the following sequence: group 1: intensive oral care programme (IOCP); group 2: 0.12% chlorhexidine mouthrinse; group 3: 0.03% triclosan mouthrinse; group 4: low-level laser therapy (LLLT). Oral mucositis was evaluated on the 7th and 14th days by means of the Oral Mucositis Assessment Scale. Their conclusions were that the IOCP and LLLT approaches showed positive results to prevent oral mucositis and should be further investigated.

Birang et al6 evaluated the effect of five different antimicrobial methods in reducing bacteria adhering to titanium surfaces. These were, the Erbium: Yttrium Aluminum Garnet ( Er-YAG) laser, plastic curette, 0.12% chlorhexidine, aPDT, and 810nm diode laser. Their conclusions were that all five mechanicals (plastic curette), chemical (CHX), laser (810nm diode and Er: YAG), and aPDT methods could reduce oral biofilms from roughed surfaces of titanium discs. Er:YAG laser and plastic curette had the highest and the lowest effects respectively.

Mendez and coworkers7 evaluated the antimicrobial effect of curcumin-ethylenediaminetetraacetic acid (EDTA)-mediated aPDT on the vitality of intact biofilms of dentin caries microcosms. They concluded that the association of curcumin and EDTA improved the antimicrobial effect of aPDT on dentin caries microcosms, resulting in the application of lower light densities with deeper penetration of biofilms.

Alasqah, Mohammed N8 carried out a systematic review of the literature regarding the antimicrobial effects of photodynamic therapy (PDT) on the multi-bacterial species and the possible surface alterations of the dental implants as a result of PDT. The review demonstrated significant reduction in the bacterial load but showed inconclusive findings regarding structural alterations on the titanium surface with the use of PDT.
